Financing Checklist for Residents

If you are considering getting financing, the following list represents some of the information SNB will review as part of your request to obtain financing:

Personal Financial Data:

  • Previous two years personal tax returns
  • Personal financial statement
  • Personal budget
  • Resume

Objectives, Goals and Structure:

  • Practice goals, objectives, vision and mission
  • Purpose and type of practice.  For example, what type of entity? i.e. LLC, PC, Sole Proprietorship—include filed state certificate, articles, bylaws, operating agreement, tax identification number
  • If existing practice, partnership/employment agreement or other legal documents
  • If existing practice, provide historical financial information (2 years)

Location and Marketing Strategy:

  • Maps, Pictures and Demographic information, i.e. the number of residents and physicians in the specific area.
  • Marketing plan - where and how will you market your practice?

Capital Requirements needed to begin practice (Actual Quotes):

  • Furnishings
  • Fixtures and Leasehold improvements
  • Equipment
  • Signage
  • Supplies
  • Working Capital
  • Other Permanent Costs

Projections:

  • Two years of projected income statements, including projected monthly expenses

Other Legal Information:

  • Leases
  • Contracts
  • Commitment Letters
  • Hospital Contracts/ Staff Acceptance Letters

Risk Management:

  • Life Insurance in amount of practice debt (at minimum)
  • Disability Insurance on Professional
  • Business Overhead Insurance
